WHAT IS MEDICAL ONCOLOGY
Medical Oncology is the science of treating and curing cancer with anticancer drugs. Anticancer drugs are of various types like chemotherapy, hormonal therapy, immunotherapy, targeted monoclonal antibodies etc. These act on the cancer cells to either kill them directly or indirectly. A combined treatment using anticancer drugs along with surgery and/or radiotherapy achieves a greater therapeutic effect.

The department of medical oncology is an integral part of reputed 500 bedded Multi-Specialty and Super-specialty Hospital in Northern India, equipped with Ultra modern and state-of-the-art facilities for Comprehensive Cancer Care - right from OPD Consultations and routine cancer health check-ups to Chemotherapy day care and Intensive Cancer Care and Specialized Neutropenia care.
The department of medical oncology is responsible for diagnosing and treating all cases of hematological malignancies as well as giving chemotherapy to majority of the solid tumors in various settings. Medical oncology is the subspecialty of oncology which deals with the medical management of cancer. A medical oncologist has knowledge of all aspects of the treatment of cancer including chemotherapy, surgery, radiation and biological therapy. The specialist determines the proper choice of drugs, dosage, and schedule of the drugs to be given. This is called, protocol making, which is a highly skilled job. It involves patient's full diagnostic work-up, staging, and assessing patients fitness for receiving a particular chemotherapy protocol.
FACILITIES & SERVICES
CHEMOTHERAPY DAY CARE:
7 bedded day care chemotherapy centre on Lower ground floor of the hospital, run by a fully trained staff comprised of 3 chemotherapy sisters under supervision of oncologist. It is utilized for chemotherapy administrastion on the day care basis, Intrathecal chemotherapy administration, diagnostic and therapuetic procedures like pleural, ascitic tapping etc. and catheter care clinic.
The department boasts of a specialized ward for day care chemotherapy administration. Chemotherapy and targeted or immunotherapy is administered through infusion pumps under round the clock supervision of cancer care nurses trained in chemotherapy. This day care administration allows the patient to walk home the same day after chemotherapy infusion saving both time and resources.

Readily available appointments for the outpatient services all 6 days of working week, run by 3 medical oncologists.

How does radiation therapy work ?
Radiation therapy uses a machine that directs high energy rays at the cancer site from outside the body. The radiation may be in form of X-Rays, Gamma Rays, Electron beam or particle therapy.

CATHETER CARE CLINIC:
Hickman catheter and hickman port care being run and managed by specially trained nursing staff and chemotherapy sisters, supervised by medical oncologists.
SPECIAL FACILITIES
Chemotherapy: intraperitoneal, intra-arterial, pleurodesis, Target chemotherapy: The newer molecules currently being applied imatinib for CML, Rituximab for CD20 + lymphomas, Geftinib for lung cancer and few other indications, Herceptin for breast cancer, Bevacuzimab and cetuximab in colorectal cancers, Velcade for multiple myeloma.

A Hickman Line
A hickman line is a soft plastic tube that is tunneled beneath the skin and placed in a large vein.
The catheter can be used to give fluids and drugs. It can also be used for taking blood samples. The benefits of having the catheter are that it will save you from having repeated needle pricks during your treatment.
You will have a double or triple lumen catheter. Each lumen will have a clamp in place, which is used to close the catheter when it is not in use.
How is the Hickman Line inserted?
Your hickman line will be inserted using a local or general anaesthetic.
During the procedure you will have two cuts made in your chest, one to tunnel the catheter and the other near the collarbone. To insert it into your vein you will have two stitches. Usually the top stitch will be removed around 7-10 days post insertion and the bottom stitch is kept in place for 2-3 weeks.
Your catheter will need to be flushed once a week and the site will require daily cleansing.
Many patients prefer to put their line into a little purse around their neck to stop it from pulling.
A PICC Line
A PICC line is a device called a peripherally inserted central venous catheter, which is inserted into a main vein in the middle of your arm situated in the ante cubital fossa.
The PICC line looks like a long cannula.
How is the PICC Line inserted into the vein?
The insertion involves a minor surgical procedure, which will be done either in the theatre or in a treatment room. The PICC line will be placed by your oncology sister, an anaethetist or by a radiologist. The procedure usually takes up to an hour and the nurses are able to access the device on each visit. Following the procedure you will be required to have a chest x-ray to ensure the correct positioning of the line.
PICC lines avoid the need for repeated venepuncture or cannulation.
Your line can be used for chemotherapy administration, for taking blood and for receiving intravenous hydration fluids.
A PICC Line requires only one cannula being inserted into one of your veins and this will remain in place for the duration of your treatment.
Your PICC Line will require a new dressing and a flush once a week. You will need to cover your PICC whilst showering and will not be able to swim whilst it is in situ.
A portacath is an implantable port device, which is positioned completely under the skin and inserted into the subclavian vein and avoid the need for repeated venepuncture or cannulation. It is used for certain patient, not all, who may require regular drug administration, often as an inpatient.
How is the portacath inserted into the vein?
The portacath will be inserted under general anaesthetic in the operating theatre. You will be in hospital for a few hours and you will be able to go home afterwards. The portacath will be positioned in your chest. You may experience some initial discomfort around the insertion site but this will go after a few hours. Your portacath can be used for chemotherapy administration, for taking blood and receiving hydration fluids. A portacath requires only one needle being inserted into the middle of the circumference of the portacath. All implanted devices must be accessed aseptically by the nursing staff. You will be able to shower and go swimming as normal.
